Leaving Hospital

Winter warm packs are available for eligible patients being discharged from our care.

We have an option of three packs. These are:

• Pack one - this pack consists of blankets, hats, scarves, and socks. Ideal for patients' in-between residences such as home to care or with no place of residence.
• Pack two - this pack consists of food with nutritional value.
• Pack three - a combination of pack one and two.

Please check with you healthcare professional to see if you meet the criteria to receive a pack this winter.
