Age UK Wakefield District - Hospital Transport Support Service

Hospital Transport Support Service provides supported transport for patients aged 50 and over who have been discharged from Pinderfields, Pontefract or Dewsbury hospitals and have a Wakefield GP. The patient must be weight barring and referred by the hospital staff or an health professional.
